Overview
The post holder will be employed by Greater Manchester Mental Health NHS Foundation Trust to provide 2 to 4 sessions to the Physical Healthcare Hubs (PHU), Referral assessment hubs (RAH), Neighbourhood (NHT)/Living Well service. The post holder will be expected to work independently to provide secondary mental health provision for clinical and medical needs of the patients on psychotropic medication. RAH/PHU/NHT works with people who are meeting the threshold for secondary care mental health teams, or who have declined/not responded to primary care mental health interventions either due to levels of complexity or uncertainty regarding their clinical presentation/additional needs.
Responsibilities
* To provide a comprehensive mental health assessment, this will involve taking a full psychiatric history and mental state examination.
* To develop a management plan to address the mental health needs which may include medication and signposting to other services.
* To provide a comprehensive risk assessment and risk management plan.
* To record all information on the electronic patient record system.
* To advise regarding any medical considerations.
* To provide follow-up appointments to monitor health and risk.
* To identify any physical health needs that may require further investigation from patients’ own GP.
* For patients with complex needs and high risk to liaise directly with secondary mental health services across services in Manchester.
* To discharge patients when no further intervention is required.
