We are seeking a candidate who is committed to supporting and working with patients who have a diagnosis of Diabetes, as part of the Long Term Conditions Service planned care provision. You will join our Specialist Nurse led team, working in partnership with both the local hospitals and primary care across East Staffordshire.
The role includes supporting reviews of Diabetes treatment and medication plans that are put in place for patients at their homes by the specialist nurses. Providing continued planning and implementation of holistic packages of care with the MDT, to support and enable Diabetes patients and their carer's to live the best quality of life as possible.
Working alongside highly skilled Senior Diabetic Nurses who are autonomous non-medical prescribers, you will provide community domiciliary care visit support, monitoring patients who are under our care. The main aim of the Diabetes nursing role is to provide an intense level of care to optimise therapies, support the individual to manage their condition, preventing unnecessary admissions to hospital.
You will need to be a NMC registered nurse who has a real interest in the nursing management and support of patients with Diabetes. You must have a desire to work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes. We are looking for candidates with good core clinical observation skills.
Main duties of the job
Provide an optimal level of care for patients with Diabetes, by demonstrating an understanding of the underlying pathophysiology of Diabetes, mechanisms of therapeutic intervention to the condition, and the significance of physical and psychological changes within the patient.
Responsible for reviewing, implementing and evaluating programmes of care, which will support the patient by providing an equitable, efficient, effective high quality and safe Diabetes service.
Work in partnership with patients to empower them to make informed choices about their health and healthcare.
Assist in the delivery of seamless care through the use of integrated care pathways in collaboration with GP's, Consultants, Palliative Care providers.
Provide education and support to patients and their carers empowering them to be partners in care.
Assist with the collection, analysis and evaluation of data relating to the Diabetes service, which includes patient related outcome measures.
Maintain personal & professional development in line with the NMC Code of Professional Conduct and understand accountability within the Scope of Professional Practice.
Take steps to safe guard the welfare, health and safety of all staff, patients and their families according to local policies.
Act as role model in the delivery of all aspects of Diabetes patient care.
