The Old School Surgery and UWE Healthcentre Frenchay are looking to appoint a highly motivated Care Navigator to enhance and support our friendly reception team.
This post offers up to 37.5 hours per week, with full and part-time options considered. The shifts will include a mixture of early mornings (starting at 7:00 am), late evenings (finishing at 7:00 pm), and occasional Saturdays. Flexibility to work across both sites is required based on business needs.

Main duties of the job
* Be the first point of contact for patients, providing support and guidance face-to-face or by phone to help them access the appropriate services.
* Handle administrative tasks such as processing documents from third parties, prescriptions, and GP requests.
* Maintain a friendly and welcoming demeanor, remaining calm and professional in challenging situations.
* Utilize our online patient access system (Klinik) to monitor requests and triage patients accordingly.
* Book appointments for GPs and other clinicians, ensuring accurate information is provided.
* Pass accurate messages and information to doctors and staff.
* Manage telephone queries and requests, acting as a liaison with GPs and clinicians.
* Participate in training sessions, including in-house and external training as required.
* Maintain confidentiality regarding patients, staff, and practice information.
* Perform any other duties reasonably expected of the role.
