Overview

Are you an innovative, organised, enthusiastic and motivated occupational therapist with a strong interest in neurology? Would you like to work in a friendly and supportive environment with a focus on CPD and further learning? We have an exciting opportunity for a Band 6 occupational therapist to join our specialist multidisciplinary community neuro rehabilitation service. The post includes the stroke pathway for clients recently discharged from stroke units and also the long term neurological conditions pathway for either progressive or acquired neurological conditions.


Responsibilities

* To provide an evidence-based assessment, treatment and advisory service to clients with neurological conditions both progressive and acquired.
* To work flexibly with all other members of the ENABLE team to provide an integrated rehabilitation service to clients in accordance with the philosophy and operational policies of the team
* To manage a varied caseload comprising clients with a range of neurological diagnosis causing neurological impairments that often include musculoskeletal, orthopaedic, cognitive and communication impairments.
* To prevent hospital admission and facilitate discharge from acute and rehabilitation hospitals.
* To participate in departmental continuous professional development, education and training, audit and research projects.
* To manage a complex caseload using evidence-based clinical reasoning, critical thinking, reflection and analysis.
* To use a client centred, goal-orientated rehabilitation approach.
* To participate in departmental clinical supervision that is responsive and personalised which can take the form of formal training, clinical reasoning, peer review, case reviews and reflective practise.
* To work flexibly & support other community therapy teams if required.
* To undertake a range of routine and specialist assessments as an autonomous practitioner (on an individual or group basis) using standardised assessments and outcome measures where appropriate.
* To have a comprehensive knowledge of anatomy, neuro-anatomy, and links with impairments and how they can impact on function; to educate others when required (e.g., post stroke impairments and impact on function).
* To form and negotiate patient-centred goals with clients, carers/relatives, and other health professionals, ensuring these are reviewed and modified as appropriate.
* To deliver evidence-based treatment programmes within a team approach in a variety of settings, ensuring safe handling, education, exercise prescription and self-management resources comply with current guidelines and are communicated appropriately.
* To devise and manage a tailor-made treatment plan addressing functional rehabilitation, vocational rehabilitation, equipment provision, orthotic assessment, community-based activities, cognitive/memory assessment and treatment, health promotion, education and psychosocial needs.
* To work collaboratively within a multidisciplinary team and support sign-posting activities across multiple statutory and voluntary organisations.
* To manage a caseload of clients with complex needs with supervision and guidance from senior staff, monitoring and adjusting interventions within a multi-disciplinary framework and assuming a key worker role as indicated.
* To ensure the client understands the clinical reasoning process and management plan and is supported to engage in self-management where possible.
* To advocate on behalf of clients and carers by adopting a client-centred approach to support meaningful participation in life roles within the community.
* To modify the client’s therapy plan following ongoing reassessment and risk assessment and to support discharge planning and follow-up needs.
* To represent the occupational therapy service at multi-disciplinary team meetings and participate in health promotion and health education initiatives in the locality.
* To manage clinical risk within own and team caseload and delegate appropriately to rehabilitation assistants with support from senior staff.


Education/ Qualifications

* Degree Occupational Therapy
* State registration with HCPC
* Relevant Post Registration courses or in-house training
* Membership and/or involvement in National special interest group


Experience

* Previous experience of working in the UK health service or a similar health economy as an Occupational Therapist
* Significant post-registration experience at band 5 or band 6 or equivalent
* Experience in Neurology Community or related areas
* Experience of supervising students or working with learners


Desirable criteria

* Management experience of departmental duties
* Experience in care of Older people
