We are seeking a highly motivated and compassionate Band 5 Community Staff Nurse to join our dynamic and supportive District Nursing team. This is an exciting opportunity to provide high-quality, patient-centred nursing care within the homes of individuals across RCT and the surrounding areas.
Responsibilities
* Assess, plan, implement and evaluate holistic nursing care for a diverse caseload of patients, promoting independence and well‑being.
* Manage own caseload autonomously while collaborating closely with multidisciplinary teams including GPs, social workers, therapists and other healthcare professionals.
* Conduct comprehensive assessment and individualised care planning.
* Provide diverse clinical interventions such as wound care, medication administration, catheter care and palliative support.
* Prioritise visits, document accurately and ensure safe, effective, compassionate care.
* Communicate effectively with patients, families and multidisciplinary teams for integrated care.
* Apply strong problem‑solving, health promotion, safeguarding awareness and continuous professional development.
Qualifications
• Registered Nurse (RN) with Band 5 qualification.
• Excellent clinical skills, strong communication abilities and a commitment to delivering empathetic community care.
Desirable Skills
Welsh Skills Desirable: Level 3 (CEFR B2) or above in speaking and listening. Not essential but an advantage.
Employment Details
Fixed term/secondment for 12 months until July 2027 covering maternity leave. Secondment: permission from current line manager required before applying.
