We’re looking for motivated, people‑focused Community Health Navigators who thrive in a fast‑paced environment. As the crucial first point of contact for all internal and external calls, you’ll provide a calm, professional, and compassionate service to patients, visitors, and staff helping them get the right support at the right time. This role involves overnight shifts, working from 12:00am to 08:00am. There is the opportunity for flexible home working once you are fully confident in the role.
What You’ll Be Doing
* Act as the professional, welcoming first point of contact for all incoming calls.
* Manage enquiries efficiently and enter accurate patient information into clinical systems.
* Assess the urgency of requests and ensure timely, appropriate responses.
* Handle Major Incident alerts by contacting key personnel and accurately recording actions taken.
* Communicate with external partners to support seamless coordination of care.
* Use switchboard and IT systems confidently while providing essential administrative support.
* Monitor fire alarms, CCTV, and security systems to help maintain a safe environment.

Package Description
As an Out‑of‑Hours Community Health Navigator, you’ll be part of our valued team based at St Martins Hospital, Bath, BA2 5RP. You will receive a salary of £25,500 - £27,098 FTE with access to our group pension, free tea, coffee and milk at your base location, and a range of benefits including rewards, cashback, e‑learning, and care coaching.
