An exciting opportunity has arisen for an experienced, proactive, and self‑motivated physiotherapist with excellent caseload management and communication skills to work within the Community Response Service Physiotherapy team in the Blyth locality.
This role aims to enable patients with a wide variety of conditions to be as independent as possible within their own environment by providing specialist physiotherapy assessment and interventions.
Responsibilities
* Triage and prioritise new referrals received into the service.
* Responsibility for a designated varied caseload of patients, organising this effectively and efficiently with regard to clinical priorities and use of time.
* Undertake a comprehensive assessment of patients including those with diverse or complex presentations using clinical reasoning skills.
* Support patients with individualised goal setting, education and care plans to work towards fulfilling their potential in a goal‑oriented way, working in partnership with patients, relatives, reablement staff, and external agencies.
* Complete all required training and ensure own continued professional development.
* Provide line management, supervision, support, guidance and training to Technical Instructors and students.
Qualifications & Experience
The successful candidate will need to be flexible in their approach and should have broad experience within musculoskeletal, falls, neurological, and general elderly frail conditions, including multiple comorbidities. They must be able to work within an integrated therapeutic approach to client intervention.
