4 months contract with a Local Authority Job Summary: • Sefton Council is seeking a dedicated and compassionate Personal Advisor to join the Care Experienced Team within Children’s Social Care. • This is a temporary 16-week role providing support, guidance, and practical assistance to young people leaving care. • The successful candidate will play a key role in helping care-experienced young people transition into independence, ensuring their health, education, housing, and emotional needs are met. Key Duties/Accountabilities (Sample): • Provide advice, guidance, and practical support to care-experienced young people to promote independence and well-being. • Participate in the assessment, preparation, and review of Pathway Plans, ensuring young people are fully involved in planning their future. • Coordinate access to services, encouraging young people to engage with education, training, employment, and health support. • Monitor and record progress and well-being through regular contact and accurate case recording on ICS and other systems. • Support young people to maintain positive health, financial stability, and suitable accommodation. • Identify risks and safeguarding issues, reporting concerns in line with council procedures and legislation. • Work collaboratively with multi-agency partners — including health, housing, education, and voluntary organisations — to deliver joined-up support. • Contribute to team meetings, supervision, and service development, promoting continuous improvement and high standards of practice. Skills/Experience: • Previous experience supporting young people leaving care or working within children’s social care, youth services, or a related field. • Strong understanding of safeguarding, risk assessment, and child protection procedures. • Excellent communication and interpersonal skills, with the ability to build trust and rapport with young people. • Knowledge of Pathway Planning, Care Leaver legislation, and the challenges faced by care-experienced young people. • Ability to work independently and as part of a multi-disciplinary team. • Strong organisational and record-keeping skills, including experience using case management systems (e.g. ICS). • Empathy, resilience, and a genuine commitment to improving outcomes for vulnerable young people. • A valid UK driving licence and access to a vehicle (desirable).
