CHC Nurse

An exciting opportunity hasarisen for an experienced and highly motivated Registered learning DisabilityNurse (RNLD) Registered Mental health Nurse (RMN) or Registered General Nurse(RGN) to work as a CHC Nurse Assessor for Gloucestershire ICB.
We are looking for anindividual who has excellent assessment and communication skills. Thesuccessful applicant will need to demonstrate the ability to work both as partof a team and also be able to work autonomously across agencies.
As a Band 6 CHC Nurse Assessoryou will be responsible for determining the eligibility for NHS ContinuingHealthcare funding through new assessments, review assessments and appeals, inaccordance with the National Continuing Health Care Framework. The role attimes involves case management of individuals that are eligible for NHS CHCfunding.
Main duties of the job
Nurse Assessors are expectedto:
Undertake nursing careassessments including Continuing Health Care (CHC) assessments
Review Funded Nursing Care(FNC) and CHC packages
Have a knowledge of the CHCFramework
Be highly motivated and keento learn/develop (training in respect of the NHS CHC Framework and itsapplication will be provided as part of induction);
Be accustomed to a high degreeof autonomy on a day to day basis;
Have excellent verbal andwritten communication skills, with attention to detail;
Have excellent peoplemanagement skills with experience of difficult situations and challengingindividuals and the ability to manage these effectively and calmly;
Be comfortable working with awide variety of ages and individuals with very different clinical conditions.
Have excellent time managementskills and thrive on working at pace.
Have excellent knowledge ofMicrosoft office and have intermediate typing skills.
Be comfortable with changes atshort notice
Be able to travel daily acrossthe county

Job responsibilities
To implement the National Framework for ContinuingHealthcare (CHC) and Funded Nursing Care (FNC) in a timely, efficient andtransparent manner and in conjunction with the individual /family andMultidisciplinary Team.
To provide high quality holistic assessments andreassessments to enable the application of the National Framework and toidentify the level of care needs and to make recommendations for thecommissioning of care.
To support thejoint working with all organisations in relation to the relevant hospital dischargepathways under the discharge to assess model (4 pathways).
Person Specification
Qualifications
* Relevant Degree/evidence of being able to work at that level.
What other skills are needed to undertake the role e.g. Oral and Written Communication skills, Computer skills
* Experience of caseload management.
* To be able to manage a caseload of patients with complex needs.
* Ability to undertake complex nursing assessments.
* Able to monitor high standards of care.
* Able to direct and co-ordinate a programme of work, collaboratively or autonomously.
* Knowledge of legal issues regarding NHS funding.
* Fully aware of NICE guidelines for the provision of specialist equipment and Long Term Conditions.
* Knowledge of promotion of continence.
* IT experience.
* Highly developed communication skills, both verbal and written, including the writing of comprehensive reports.
Length and Nature of Experience
* At least three years registered nurse experience.
* Working knowledge of the National Framework for CHC and FNC.
* Working knowledge of Primary Care Services.
* Palliative Care experience.
* Community/ Care Home experience.
What other Professional or Specialist Knowledge is required e.g. Post Graduate qualifications/ courses, Research Methods
* Evidence of continuing professional development at degree level.
* Community Nurse Qualification.
* Teaching and assessing-998 or equivalent.
* ECDL.
* Continence course.
* Tissue Viability course.
Other - include any physical skills e.g. dexterity, use of equipment, mobility as well as any physical demands, driving
* Ability to fulfil the physical requirements of the post.
* Must be proficient in clinical skills required for area of practice including knowledge of equipment and continence products.
* Ability to travel to all areas of Gloucestershire and occasionally out of county.
