University Hospitals Dorset (UHD) is seeking an Advanced Clinical Practitioner (ACP) to join our forward-thinking Stroke and Neuro Service, working across our Stroke Unit and our Early Supported Discharge (ESD) Teams -- a leading service that transforms patient recovery by delivering specialist care at home.

Main duties of the job

Job responsibilities
The Candidate Were Looking For:
 * A qualified ACP with a passion for neuro and stroke rehabilitation and for implementing new and innovative ways of working
 * A strong clinical decision-making skills and a confident autonomous practitioner
 * An excellent communicator and team player, able to inspire and motivate patients and colleagues alike
 * Committed to continuous learning and service development
 * Energetic, adaptable, and proactive ready to thrive across the dynamic inpatient and community settings
The post holder will contribute to both the Stroke Unit and the Early Supported Discharge Team, with duties divided on a 60:40 basis, supporting seamless patient care across the stroke pathway.
Person Specification
Shortlisting
 * HCPC Registered AHP
 * Extensive post graduate clinical stroke/neuro experience in an acute/sub acute stroke team.
 * Management and or Leadership course relevant to healthcare environment.
 * Supplementary or independent prescriber.
Shortlisting and interview
 * Demonstrable experience of managing patient's care at an advanced level of practice with specific knowledge and skills in a field of expertise within the stroke and neuro rehab context.
 * Publication/Conference experience.
