<div><h3>Role Overview</h3><p>We are looking for a <b>Support Practitioner</b> to support a young man with autism in his own home.</p><h3>Responsibilities</h3><ul><li>Support him with daily living tasks, household chores, medication management, personal care and finances.</li><li>Assist him with meals, shopping, transport and other daily activities.</li><li>Encourage participation in his leisure activities and help him to explore new opportunities.</li><li>Maintain accurate records, report incidents and communicate with the care team.</li><li>Provide companionship, encouragement and help him work towards personal goals.</li></ul><h3>Shift Patterns</h3><p>24/7 tailored support. Shifts vary by day, with changes on Tuesday, Wednesday and Friday due to day‑service commitments.
