Community Sister/ Charge nurse/ District Nurse
An exciting opportunity has arisen to join our multidisciplinary community team as a Community Sister/ Charge Nurse. This role involves coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The position is that of a caseload holder, providing clinical case management for patients with complex health needs who are at risk of deterioration, which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of hospital stay.
We currently have vacancies at Brighton General Hospital for Brighton & Hove East and South community nursing teams.
Main duties of the job
Community Sister/Charge Nurse
Some of your main duties will include (but not limited to):
1. In conjunction with other key professionals, you will be accountable for a patient caseload and delegate responsibility to other professionals within the multidisciplinary team.
2. Apply advanced assessment of physical, mental, and social care needs to provide clinical care and health-promoting interventions that help avoid unnecessary hospital admission.
3. Interpret clinical signs, symptoms, and diagnostic tests to develop treatment plans and/or treatments in conjunction with medical practitioners as appropriate. Provide advanced clinical advice to the wider healthcare team to improve patient care outcomes.
4. Enable patients to be as independent as possible, facilitating a range of self-management strategies.

Job responsibilities
Some of your key responsibilities will include (but not limited to):
* Case manage a designated patient caseload and manage/delegate care delivery by the integrated team
* Support the team leader and clinical lead in providing, monitoring, and improving the quality of care, including risk management, compliance with CQC requirements, and clinical audits
* Oversee clinical areas, facilitating junior staff to lead specific areas
* Deputize for the Clinical Nurse Lead as necessary; assist in recruitment, appraisal, risk assessment, complaints, and performance management processes
* Provide clinical supervision/peer review, active clinical leadership, and mentorship to junior colleagues
* Be aware of the Trust's financial position and implications on service delivery; oversee equipment and supplies, manage clinical stock, and maintain equipment per Trust policy
* Review practice roles and participate in appraisal to identify development needs and create personal development plans
* Act as a practice assessor for staff and students
