The Service
Hospital in the Home (HITH) is Cabrini’s acute outreach service, providing hospital-level treatment to patients in the comfort of their own homes. The service also delivers care to Post Acute Care and Chemotherapy patients under our Cabrini at Home program.
The Role
We are currently seeking a Senior HITH Nurse to join our team on a part time basis 0.6 FTE basis, with consideration of alternative FTE options for the right candidate.
You’ll work across a rotating roster, including some on-call shifts, to ensure seamless, high-quality care in the home setting. This senior clinical position offers the opportunity to work both autonomously and collaboratively with physicians, specialists, and other health professionals to deliver exceptional, patient-focused care.
Key Responsibilities:
6. Develop and implement comprehensive care plans tailored to each patient’s needs
7. Monitor patient progress and ensure continuity of care
8. Conduct clinical assessments, administer treatments, and manage patient care
9. Provide education to patients and families on health management and recovery

About you
To be successful in this role, you will possess:
11. Current AHPRA registration as a Registered Nurse
12. Proven acute and community nursing experience
13. Strong clinical skills in wound care, IV therapy, and CVC management
14. Ability to work independently and respond to clinical deterioration
15. Excellent communication and documentation skills
16. Commitment to quality improvement and ongoing professional development
17. A valid driver’s licence and Working with Children Check
