Main duties of the job

The focus of this Higher Specialty Trainee level fellowship post is to lead on project that aims to further develop the hospital at home team.

Hospital at home is an initiative that has been developed at Darent Valley Hospital to provide short-term, acute care within patients' homes (including residential and care homes) to manage acute problems and safely reduce the need for in-patient hospital care. It is led by a senior multidisciplinary team who have expertise in hospital based acute care, and aims to manage the holistic needs of the patients.

This fellowship will enable a senior trainee to work with the multidisciplinary team to further develop the project. This will embed service development and leadership skills, working with the wider hospital leadership team. This would include being a stakeholder in the hospital-wide programme to improve patient flow, attendance at patient safety reporting meetings and complaints responses.

Job responsibilities

Three days non-clinical 0.6 FTE (60% minimum) for leadership and education fellowship work leading specified project work and participating in the development programme, including attendance at BSMS for the academic modules, Learning Network attendance and personal study.

The days for module attendance vary, but it is asked that Fridays are one of the non-clinical days, so that all Fellows are available on those days for contact.

Two days will be spent working with the hospital at home team clinically, involving input into virtual reviews, patient case review, home visits and administration


Person Specification


Qualifications

* oMBBS or equivalent medical qualification
* oFull GMC Registration and License to Practice at the time of appointment
* oIntercalated BSc or equivalent Higher degrees including MSc, PhD or MD
* oPublications in recognised medical or academic journals
* oAttendance at medical courses and conferences


Knowledge

* oCompetence in independent management of medical emergencies
* oEvidence of independently leading teams
* oAcademic excellence
* oAn understanding of NHS management and resources


Experience

* oEvidence of good career progression as demonstrated by employment history - at least 12 months working in the NHS
* oExperience in delivery of teaching and education.
* oEvidence of active participation in audit
* oEvidence of interest and experience in delivering teaching to medical students and trainees
* oExperience of setting up courses
* oFaculty on local or national courses
* oExperience of Quality Improvement projects
