The Role

This post is for Care Home Liaison Nurse. An exciting opportunity has arisen for a Nurse Team leader within the NE Older Peoples CMHT. The successful candidate will work collaboratively with patients, carers, the wider multi-disciplinary team and third sector organisations. A strength based, person centred approach will be essential to assessment and formulation of care plans to meet the identified needs of the patient. Implementation of the Physical health care Policy will be a critical role, providing monitoring in line with current GG&C guidelines to ensure timely and accurate record keeping. A key role will be providing support and guidance to staff Nurses and Health Care Assistants and engaging in regular MDT discussions, groupwork, along with the administration of depot medication and regular reviews of antipsychotic medication according to GG&C policy and guidelines. The role includes support and supervision to student nurses and junior staff and being part of the desk based CMHT duty rota. The role includes supporting the NTL and colleagues to implement local and national policies and to provide nursing cover to the CMHT as required.


Location and Working Pattern

* Location based in Parkhead Hub
* Permanent Contract
* Full-time Position (36 hours per week)
* Shift pattern for this post are hours between 09:00 17:00


Key Responsibilities

* Functioning as Named Nurse / Named Practitioner within a multi-disciplinary team for a complex patient caseload requiring specialist mental health care undertaking care coordination where required.
* The holistic assessment of complex care needs including risk assessment, planning, implementing and evaluating specialist programmes of care for individual caseload and monitoring the nursing staff caseload.
* Carrying out, reviewing and monitoring all relevant forms of nursing care pertinent to the needs of individual patients in the least restrictive and disruptive manner as possible; focusing on improving the mental and physical well-being of patients; responding to and supporting the staff team to respond to issues pertaining to vulnerability, gender, health, domestic abuse, adult/ child protection and or self harm.
* Ensuring therapeutic staff / patient relationships involving complex / unstable care needs are met within a participative framework demonstrating principles of equality, dignity and respect accounting for spiritual and cultural diversity. Working proactively with other agencies services (ie interpreter services) to aid effective communication.
* Provide regular caseload / nurse line management supervision and Clinical Supervision to Band 6 nurses. Ensuring that systems are in place for regular caseload / nurse line management and clinical supervision provision with the nursing staff team.
* Monitoring the practice and performance of Band 6 nurses and ensuring that there are systems in place for supporting the ongoing professional and practice development.
* Provide advice regarding specialist mental health nursing matters to members of the nursing team, the CMHT and others within the wider health and social care partnership.


Essential Qualifications

* Mental Health Nurse (RN3/ RNMH) level 1 NMC registration
* Educated to minimum degree level or equivalent
* Completion of an evidence based psychological therapies / psychological approaches to care formal programme of study
* Specialist knowledge and application of mental health nursing care (including physical healthcare) for people with complex / unstable care needs
* Specialist knowledge and application of risk assessment / risk management factors impacting on metal health / mental illness / wellness and recovery for people with complex/ unstable care needs
* Evidence of active and ongoing CPD
* Evidence of Line Management Supervision / Clinical Supervision and care coordination of senior staff
* Specialist knowledge of rights, values based practice and legislation frameworks


Desirable Qualifications

* Completion of frontline leadership / management programme
* Actively working towards or above postgraduate level or equivalent
* CMHN Career Development Framework evidence of pillars of practice at Level 7 Advanced Practice (equivalent)
o Clinical practice safe effective person centred care

o Leadership – applied
o Research and development
