Job overview
Thurrock ICT, North East London Foundation Trust is seeking an enthusiastic and motivated Registered Nurse with a Specialist Practitioner qualification to work in the Integrated Community Team, as a Complex Case Manager for Palliative/End of Life.
You will need to be able to demonstrate the following essential criteria skills for the post:

The post holder will be responsible for Clinical, leadership and managerial duties in support of the Operational Lead. They will also be responsible for the identification of patients on the ICT caseload requiring palliative and approaching end of life as well as support the ICT staff delivering end of life care by developing skills around staff debrief.
The post holder may be required to line manage a team of Community Nurses and Health Care Support Workers. They will supervise and appraise staff members and students.
The post holder must possess excellent leadership and communication skills.
The post holder must hold a valid UK driving licence.
Main duties of the job
Their duties will include but not be limited to:
8. Identifying those patients on the ICT caseload who are requiring palliative care and approaching end of life.
9. Coordinate care, involving liaising with any other interested parties involved.
10. Enhance quality of life for people who are approaching End of Life, ensuring end of life pathways are in place
11. To provide assessment and treatment alongside the caseload holders for patients who are approaching End of Life and already receiving face to face care from the ICT, in a safe environment, protecting them from avoidable harm.
12. Supporting ICT staff in education and training around identification of deteriorating patients and Advanced Care Planning (ACP).
13. Undertaking required audits.
14. Providing bespoke debrief to ICT staff.
15. To improve outcomes for people who are palliative and approaching End Of Life and reduce the need for these people to attend or be admitted into hospital, supporting them to remain in their preferred place of care.
16. To ensure all patients that require a PEACE or similar document or are on and end of life pathway, have appropriate pathways in place with associated documentation and are on the appropriate register.
The post holder will support their peers and locality lead and be an effective team member whilst working on their own initiative. They will supervise staff members and students.

High Cost Area Supplement
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,303 and a maximum payment of £2,198 per annum pro rata).
