This is an exciting opportunity to join our CARA service at Leeds Irish Health & Homes (LIHH) which delivers a wide range of service responses to those most in need in our community, including the elderly, those experiencing physical and mental health challenges, financial pressures, and social and cultural isolation.
* Manage a caseload, working one-to-one with individuals to assess needs, develop support plans, and provide ongoing assistance.
* Facilitate social, cultural, and health related group activities as part of our CARA programme.
* Provide person centred support that empowers individuals to access services, build confidence, and engage in community life.
* Be responsible for collating information to meet monitoring and evaluation requirements.
We need you to have:
* An outstanding attitude and track record in supporting vulnerable people.
* A passion for community development and culturally sensitive practice.
* Excellent organisational, time management and communication skills.
* A strong record of supporting vulnerable people with compassion, dignity, and respect.
* The ability to work flexibly as part of a supportive team.
