We have an exciting opportunity within our community inpatient unit at Wokingham Community Hospital for a clinical administrator, called a Patient Flow Co‑ordinator. The post holder will be an integral member of the multi‑professional team and will support the patient flow through the admission and discharge process.
Responsibilities
* Support patient flow through admission and discharge processes.
* Attend MDT and allocation meetings, ensuring referrals are placed in a timely fashion.
* Coordinate care for safe transfers when a patient is referred from acute hospitals or home.
* Facilitate admissions and discharges, review and process referral documents.
* Communicate with patients, relatives and external services such as In Reach Team, Intermediate Care, Community Health and Social Services.
* Serve as point of contact for the service, working with Integrated Therapy/Flow Teams.
Qualifications and Experience
* NVQ4 or NVQ3 or a recognised certificate in office administration or equivalent experience.
* Experience working in a clinical or community health or social setting.
* Experience in multi‑agency collaboration across professional boundaries.
* Excellent interpersonal and IT skills.
