District Nurse/Community Nurse

Job Overview: We are looking for an experienced and motivated Band 6 Community District Nurse to join our Community Nursing Team. This is a rewarding role where you will deliver high-quality, patient-centred care to individuals in their own homes, supporting them to maintain independence and avoid unnecessary hospital admissions. You will manage a defined caseload, working both autonomously and collaboratively within a multidisciplinary team. As a Band 6 nurse, you will play a key role in clinical leadership, supporting junior staff, coordinating care, and contributing to the continuous development of the service.

Main duties of the job: Applicants will have NMC Registration and demonstrate the ability to innovate and motivate. Applicants are required to evidence a full UK driving licence and the ability to travel to a variety of sites (This criteria will be reasonably adjusted to meet the requirements of a disabled applicant). This position involves working weekends & some bank holidays.

Clinical Responsibilities

* Undertake holistic assessments of patients with complex health needs in community settings
* Plan, implement, and evaluate care, ensuring safe and effective clinical practice
* Deliver a range of nursing interventions including wound care, long‑term condition management, and end‑of‑life care
* Support hospital discharge and admission avoidance pathways


Caseload & Care Coordination

* Manage and prioritise a defined caseload, ensuring high standards of care delivery
* Act as a key worker, coordinating care across health and social care services
* Monitor and review care plans, adapting interventions as required


Leadership & Team Support

* Provide supervision, support, and mentorship to Band 5 nurses, HCAs, and students
* Contribute to daily team coordination and workload allocation
* Support the Team Leader in service delivery and development


Multidisciplinary Working

* Work closely with GPs, therapists, social services, and other agencies
* Participate in integrated team meetings and joint care planning
* Build strong relationships with patients, families, and carers


Quality & Governance

* Maintain accurate and timely clinical documentation
* Adhere to Trust policies, clinical standards, and professional guidelines
* Participate in audits, service improvement, and quality initiatives

Starting with NELFT: NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop‑in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period: This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, Goodmayes) This post also attracts payment for a High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
