Job summary
This role is offered as for30 hours per week over 3 days (Monday, Thursday and Friday) the Monday is an8pm finish. There will be sessions with the home visiting team (roughly once aweek) and the role will be predominately supporting with same day triage. Basedat our Annandale site and Knutsford Community Hospital for extended access andvisiting sessions. The ACP will have a named clinical supervisor on site andsupport from our other ARRS roles.

CLINICAL RESPONSIBILITIES
Safely manage undifferentiated and undiagnosedpresentations using data gathering, assessing for red flags, investigation,diagnosis, management, and review within their agreed scope of practice andcapabilities. This will include within the organisation setting, home visitsand care homes
Must demonstrate complex clinical reasoningskills, to use problem solving approaches to analyse and synthesiseinformation, manage medical and clinical uncertainty particularly in relationto a wide range of contexts and clinical presentations, and to includephysical, mental, and social problems
Have a sound understanding of referral pathwaysincluding routine, urgent, 2-week wait and emergency admissions Use resourcesappropriately and effectively
Manage acute/chronic presentations and medicalcomplexity
Safely and effectively use a range ofconsultation modes to include telephone, video, online and face to face
Prescribe treatment and medication safely inline with local and national guidelines (where professional regulation andtraining permits, an independent prescriber)
To offer personalised care to people andsignpost to and link with other members of the organisation or wider health andsocial care team

Essential
1. Registered as a health care practitioner with NMC, HCPC, GPhC with a minimum of 5 years post registration experience BSc level
2. Registered General Nurse with evidence of undertaking Level 6 or 7 study
3. MSc/Post Grad diploma to include modules in advanced assessment and diagnostic reasoning, complex clinical reasoning

4. Independent prescribing qualification
5. Advanced Care Practitioner MSc/Post Grad Diploma
6. Be recognised as advanced practitioner on the HEE Centre for Advancing Practice Directory either from completion of a HEE Centre accredited MSc Programme or through the Portfolio Route to recognition
