We’re hiring a Social Prescribing Advisor to join the Live Well Wakefield team, a community-based social prescribing and self-management service which supports adults aged 18+ to improve their health, wellbeing and independence.

What does a Social Prescribing Advisor do?
Advisors at Live Well Wakefield work with people at risk of reduce wellbeing and independence, providing them with social prescribing information, advice and support on a 1:1 basis. This facilitates people to access appropriate support services and can positively impact their employment status.
This role is in a multi-disciplinary environment, and includes developing community resources and networks between all sectors to improve wellbeing and reduce inequalities for adults in Wakefield District
* Experience of undertaking direct work with people to help improve health and wellbeing.
* Experience in managing a caseload and prioritising need.
* An understanding of health and social care, and employment issues for vulnerable adults and how these relate to health inequalities.
* An understanding of the importance of confidentiality and safeguarding.
* Ability to engage, motivate, and create good working relationships with a variety of local people, community groups and professionals.
* Ability to drive, access to own vehicle, and willingness to travel across the Wakefield District.
