The Alcohol Care Team is looking for an enthusiastic nurse with a passion for supporting patients with alcohol‑related conditions, who will contribute to the ongoing development of the service.
Key Responsibilities
* Support patients to access community services while working closely with referrers, multidisciplinary teams and external agencies to ensure continuity and coordination of care.
* Advocate for patients, ensuring their needs, preferences and voices are respected and central within care planning and decision‑making processes.
* Facilitate education and training for healthcare professionals, focusing on alcohol identification and screening, brief interventions, reducing stigma associated with alcohol use disorder and alcohol‑related harm, and alcohol withdrawal.
The team is continuing to evolve to meet the standards set by the Alcohol Care Team Innovation and Optimisation Network (ACTION).
