Overview
Newport Pagnell Medical Centre is looking to welcome a practice nurse on a full or part-time basis into their busy team to provide treatment room duties and ideally be experienced in long term conditions. The hours for this role can be flexible, so please apply to discuss. The post will involve working at Newport Pagnell Medical Centre, NPMC@Willen and NPMC@Kingfisher with flexible working considered. The practice holds Saturday morning clinics (0800-1230) working alongside a duty GP, managed by the nursing team on a rota basis and additional to the contracted hours. In this role, you will be responsible for the delivery of general practice nursing services to the practice population.

Responsibilities
* Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing.
* Implement and evaluate individual treatment plans for patients with known long term conditions.
* Proactively identify and manage treatment plans for patients at risk of developing long term conditions.
* Prioritise health problems and intervene appropriately in complex, urgent or emergency situations.
* Support patients to adopt health promotion strategies that encourage healthy living.
* Provide information and advice on prescribed or over-the-counter medications, including regimens, side effects and interactions.
* Support and manage health needs of women presenting for cervical cytology consultations.
* Recognise, assess and refer patients presenting with mental health needs.
Qualifications
Essential
* Current NMC registration with an active PIN.
* Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience/qualifications.
Experience
* Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
* Compassionate manner with an empathetic approach and respect for patients, relatives and carers as well as colleagues.
* Knowledge and experience of supporting long-term conditions.
* Insight and understanding of current issues in nursing and the wider NHS.
Desirable
* Long-term conditions experience desirable.
