Job summary
Newport Pagnell Medical Centre are recruiting a practice nurse. We are keen to hear from those seeking both full time and part time hours for this role. The practice holds Saturday morning clinics 0800-1230 working alongside a duty GP - these are managed by the nursing team on a rota basis and will be additional to the contracted hours.
In this role, you will be responsible for the delivery of general practice nursing services to the practice population.
Main duties of the job
1. Assess, plan, develop, implement and evaluate programmes to promote health and wellbeing.
2. Implement and evaluate individual treatment plans for patients with a known long term condition.
3. Proactively identify and manage as appropriate, treatment plans for patients at risk of developing a long term condition.
4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
5. Support patients to adopt health promotion strategies that encourage patients to live healthily.
6. Provide information and advice n prescribed or over the counter medication on medication regimens, side effects and interactions.
7. Support and manage health needs of women presenting for cervical cytology consultations.
8. Recognise, assess and refer patients presenting with mental health needs. Implement and participate in vaccination and immunisation programmes for both adults and children.

We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area.
The practice nursing team forms part of our integrated nursing team working alongside our community nursing team, community matrons, urgent care team and specialist nursing team.

9. Current NMC registration with an active PIN.
10. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications
11. Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Experience

12. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
13. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
14. Knowledge, and experience of its application, in supporting long-term conditions.
15. Insight and understanding of current issues in nursing and the wider NHS.
