Overview
Are you passionate about patient safety and driven to make a meaningful difference? We are looking for someone who brings curiosity, compassion and strong analytical skills to join us as our Patient Safety and Quality Officer (Band 7).
In this role, you will play a key part in embedding the Patient Safety Incident Response Framework (PSIRF) across the Division. You will lead and coordinate Patient Safety Incident Investigations (PSIIs) and learning responses, support staff and families through sensitive and complex situations, and help turn insights into practical improvements that enhance care.
You will work closely with the Patient Safety Lead, divisional governance teams and colleagues across theatres, wards and specialist services, building a positive safety culture and strengthening how we learn from experience.
We are looking for someone with experience in patient safety, systems thinking, human factors or quality improvement – someone confident in influencing, skilled in listening and committed to delivering high-quality, compassionate care.
Key Responsibilities
* Embed the Patient Safety Incident Response Framework (PSIRF) and lead the coordination of PSIIs and learning responses.
* Analyse incidents, identify contributory factors, produce high‑quality reports, and support teams to develop sustainable safety actions.
* Provide compassionate engagement with staff, patients and families, support Duty of Candour and maintain an open, fair and supportive safety culture.
* Identify themes and risks, translate learning into actionable improvements and contribute to divisional risk registers.
* Support quality assurance, regulatory preparedness, complaints investigations and clinical effectiveness activity.
* Promote quality improvement and innovation, help teams use data to enhance care, and coordinate audit and data quality initiatives.
* Build effective relationships with clinical and operational teams, coach colleagues, line‑manage the patient safety and quality team and represent the division in governance meetings.
* Produce clear, high‑quality reports, monitor the safety agenda and contribute to digital developments that strengthen patient safety.
Qualifications & Skills
• Experience in patient safety, systems thinking, human factors or quality improvement.
• Strong analytical skills, ability to influence across professional groups and build relationships.
• Demonstrated ability to lead investigations, manage teams and deliver actionable improvements.
• Commitment to compassionate care, effective communication and a culture of learning.
