Clinical Pharmacist Independent Prescriber
The position requires a qualified pharmacist with an independentprescribing qualification, who will operate within their professional limits asa member of a multidisciplinary team
This position is up to 25 hours per week, working days negotiable
Main duties of the job
The successful candidate will collaborate closely with the GP and otherpractice clinical and non-clinical staff, playing a key role in supporting GPsand the broader primary care team to provide safe and effective care. They willprovide medication-related patient services, including direct patient care.

The successful applicant will need to beself-motivated and able to work on their own, You will have excellentorganisational, interpersonal and communication skills together with a goodworking knowledge of primary care and the challenges facing patient care in theNHS.

Job responsibilities
* Consult with patients who have one or more medical conditions that require medication optimization e.g., COPD, asthma, hypertension.
* Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments i.e., medication optimization.
* Provide GPs with recommendations for medication improvements.
* Conduct clinical medication reviews with patients and offer recommendations to the senior clinical pharmacist, nurses, and/or GP regarding prescribing and monitoring.
* Respond to medicine-related inquiries from GPs, practice staff, other healthcare teams e.g., community pharmacy, and patients.
* Follow up with patients to monitor the effects of any changes.
* Review the use of medications commonly linked to unplanned hospital admissions and readmissions through audits and individual patient reviews.
* Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines.
* Monitor practice prescribing in accordance with the local formulary and recommend to GPs which medications should be prescribed by hospital doctors (red drugs) or be subject to shared care (amber drugs).
* Audit the practices compliance with NICE technology assessment guidance.
* Comply with safeguarding policies and support the commitment to promoting welfare.
* Comply with all Bradford Moor Practice policies and stay informed about any changes.
Person Specification
Experience
* Demonstrates an understanding of, and conforms to, relevant standards of practice.
* Follows professional and organisational policies/procedures relating to performance management
* Ability to convey complex and sensitive information clearly to various audiences, such as patients
* Capable of obtaining and analyzing complex technical information
* Ability to work under pressure and meet deadlines
* Produce timely and informative reports
* Work effectively both independently and as a team member
* Self-motivated
* Competent at medication reviews, especially long term conditions
* Demonstrates ability to integrate general practice with community and hospital pharmacy teams and community groups
Professional Registration
* Mandatory registration with the General Pharmaceutical Council
* Membership Primary Care Pharmacy Association (PCPA)
* Membership of the Royal Pharmaceutical Society
Other
* Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
* Adaptable
* Work effectively independently and as a team player
* Safeguarding and other mandatory training
* Access to own transport
* Immunisation status
Qualifications
* Professional knowledge acquired through vocational masters degree in pharmacy (4 years) plus 1 year pre-registration training
* Extensive knowledge of medicines optimisation, acquired through post graduate clinical pharmacy diploma or equivalent experience or training.
* Holds an independent prescribing qualification
* Postgraduate Diploma in Clinical Pharmacy
* Successful completion of the CPPE General Practice Pharmacist Training Pathway
