About The Role

This role focuses on specialising in the development and reviews of person‑centred assessments and support plans. It supports a culture of person‑centred practice, coaches colleagues, drives outcomes‑based culture, delivers workshops, and responds to challenges through pathways, tools and quality measures.


Key Accountabilities and Responsibilities

* Work closely with colleagues to complete person‑centred assessments, collaborating with internal and external professionals to ensure robust quality support plans.
* Upskill colleagues in conducting quality assessments and reviews, and developing meaningful outcome‑driven support plans.
* Work and communicate with people we support to ensure they are at the centre of the development of assessments, support plans and outcomes.
* Collaborate with family, friends and professionals to gather detailed information that informs assessments and support plans.
* Remains involved with people supported and their vital others during the first 12 weeks, ensuring the support meets their needs in a person‑centred, outcome‑focused way.
* Lead a 12‑week review after admission to sign off or amend the package of care and support.
* Keep up to date with local and national policy guidelines in the sector.
* Develop and deliver, with passion, person‑centred assessment and support planning principles sessions.
* Review current support plans, identify improvement areas, and share findings with team leaders and support staff, helping colleagues understand importance.
* Set realistic and achievable action plans to improve quality of support plans.
* Drive cultural change to embed person‑centred practices reflecting the mission, vision and values.
* Write reports and presentations that reflect completed work, focusing on key areas and trends.
* Engage in continuous learning to enhance skills and competencies.
* Identify and develop pathways, assessment tools and quality measures for challenging areas in partnership with quality partners.
* Support colleagues in the continuous development and improvement of operational practice.


Skills and Knowledge

* Communicates clearly and concisely, orally and in writing.
* Extracts quality information through questioning and conversational style.
* Works well as part of a team and independently.
* Manages own workload effectively.
* Possesses effective interpersonal skills and a positive attitude towards people we support.
* Demonstrates a working knowledge of MCA (2005).
* Commits to self‑development and development of others.
* Develops and maintains effective working relationships.
* Computer literate.
* Prioritises and multi‑tasks.
* Adapts to changing methods of working.
* Understands confidentiality and consent.
* Good understanding of social care regulatory requirements.
* Excellent understanding of safeguarding guidelines.
* Understands how to set and achieve evidenced outcomes.


Values and Attitudes

* Minimum of 2 years’ experience working in health and social care.
* Minimum of 2 years’ experience completing assessments and support plans.
* Experience in developing risk assessments and management plans.


Qualifications & Training

A Level 3 Qualification in Health & Social Care, or a Level 3 qualification in a related area.


Other Requirements

* Car Owner / Driver.
* Flexible with working hours and willing to travel, including overnight if required.
