Description
Swansea Council is seeking a committed and experienced Social Worker to join our Community Mental Health Team (CMHT). This is a fantastic opportunity to contribute to a multidisciplinary team dedicated to providing high-quality mental health support across the community. Based in Swansea, this role involves a hybrid of office-based work, home visits, and hospital assessments.
Responsibilities
* Deliver statutory mental health services in line with the Social Services and Well-being (Wales) Act 2014 and the Mental Health Act 1983.
* Undertake assessments and develop person-centred care plans, working in partnership with service users, families, and carers.
* Manage complex cases and act as care coordinator, ensuring effective risk management and safeguarding interventions.
* Prepare and present reports, including for Mental Health Tribunals.
* Work collaboratively with NHS colleagues and other professionals to support recovery-focused care.
* Maintain accurate, up-to-date records and contribute to service development.
