If you have a strong passion for providing compassionate care to patients and a desire to positively impact individuals' lives, Clarkson Surgery is looking for an experienced Practice Nurse, ideally with Diabetes experience to join our Clinical Team. The new colleague will work 37.5 hours per week Mon-Fri with flexibility possible for the right candidate (with a 1 hr unpaid break).
We are looking for an enthusiastic, flexible, dedicated and highly motivated individual to join our forward thinking team at Clarkson Surgery. We are a friendly patient focused practice serving our local community. Our list size is approximately 9,500 patients. Our Practice team comprises of GPs, clinical and nurse practitioners, practice nurses, healthcare assistants, a pharmacist team, physiotherapists, care co-ordinator, social prescribers and mental health support along with our reception, admin and dispensary teams. We are looking to develop our patient services further and hope our new colleague can help us do this.
Job responsibilities
To provide a high quality clinical nursing service and support to other professionals and to patients of the practice including treatment, screening, chronic disease management and advice within the professional competence of the post holder. Support and work effectively within the clinical team. All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regard to the various practice protocols. The nurse is part of a nursing team responsible for ensuring that the practice provides an optimum service of nursing care relevant to the practice population.
* Meeting patients nursing care needs as identified, or where care has been transferred to them by a GP or other clinical colleague.
* Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
* Managing the nursing care of patients with chronic diseases, liaising with admin colleagues as necessary.
* The following clinics are offered at Clarkson Surgery by our Practice Nursing Team: Asthma/ COPD, HRT reviews, Travel advice and vaccinations, Diabetes, Cardiovascular Disease, Cervical Smears, Family Planning, immunisations, Amending or adding to services as needs are identified.
* Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to ensure continuity of care where necessary.
* Immunisations, ECGs, Dopplers
* Screening – undertake screening including: BP checks, height, weight, BMI, Urinalysis, smoking status. Undertake venepuncture according to practice protocol
* Infection control: adhering to infection control procedures according to local and national guidelines
* Emergency Management: be competent in anaphylaxis and resuscitation techniques; attend updates when required
* Womens Health: have a working knowledge of the national and local Cervical Cytology and breast screening programmes; discuss breast self-awareness and provide supporting literature to patients
* General Duties: maintain accurate patient records on the computer; collect data and participate in regular practice audits and research projects; log and dispatch specimens as appropriate; assist the Practice Nurse team in the provision of Health Promotion and Chronic Disease Management
* Education: maintain continuous professional development; participate in training of other personnel; contribute to the Practice and Professional Development Plan
* Information and IT: facilitate good communication systems within the practice with the use of computerised messages; receive and understand information from outside agencies and bring to the notice of clinicians where necessary; undertake training and attend computer data input and retrieval meetings; be available to deal with patient queries beyond the reception team; ensure that formal complaints are passed to the Practice Manager for action
* Additional skills: prioritise and balance routine administrative tasks with management issues; make balanced and appropriate decisions; contribute to future practice policies and growth; implement new ideas to improve performance; communicate effectively with Doctors, Practice Manager, the team, colleagues and outside agencies; work towards cost effectiveness; have working knowledge of clinical software and computer/medical equipment
Professional Registration: You will be expected to maintain your own annual registration with the NMC and be conversant with the NMC code of Professional Conduct and understand the importance of professional accountability.
Qualifications
* Registered nurse with a minimum of 2 years post-registration
* Up-to-date validation with the NMC
* Basic/advanced Certificate of Life Support
* Chronic disease management awareness/skills
* Working experience of supporting diabetes patients
* Willingness to learn and acquire new skills
* Insight and understanding of current issues in nursing and Primary Care
* Evidence of appropriate knowledge base
* Demonstrate an understanding of audits
* Understanding and awareness of health and safety issues
* Good interpersonal skills, both verbal and written
* Evidence of problem-solving and decision-making skills
* Good team player and ability to get on with others at all levels
* Ability to be reflective, accept feedback and act constructively
* Ability to use initiative as appropriate
* Willingness to undertake further study as appropriate to the role
* Ability to cover colleagues absence
