We are looking for a Qualified and Experienced Social Worker to join the CHAD team.
Working collaboratively with colleagues across a range of agencies and council directorates from Health, Education, the voluntary sector and Adult Services. You will be offered a unique opportunity to support children and young people with physical, sensory and learning needs who may require Care and Support plans; Transition planning; Child Protection plans; Looked After Children procedures; Court proceedings.
You will be committed to the work of children with disabilities in a team focused supportive learning environment.

It is essential that you are a Qualified and Experienced Social Worker registered or eligible for registration with Social Care Wales .

Responsibilities of you as the Child Health and Disability Social Worker:
8. Ensure the best outcomes for the children and young people.
9. To work within current legislation and Council policy, procedures and practice to assess, commission and review services, including planned interventions, for a specific client group.
10. To undertake and promote the effective assessment, care planning, intervention and review for children in need of care and support, including children in need of being looked after and/or protection, including circumstances where it is necessary for the local authority to initiate legal proceedings to safeguard children.
11. To work with individuals, families, carer groups and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning.
12. Support children and young people through robust assessment, outcome focused planning and effective intervention to support them in achieving positive outcomes within their families and communities.
Requirements of you as the Child Health and Disability Social Worker:
13. Social Care Wales registration
14. A recognised Social Work qualification (Degree/DipSW/CQSW)
15. A full UK driving licence
16. DBS clearance
17. Authority to work in the UK
Location: South Wales
Salary scale: £38,– £42,/strong> dependent on experience
