Job Description
As a Social Worker in the Children's Long Term Team, you will play a crucial role in supporting some of the most vulnerable children and families in Newcastle. You will be responsible for assessing, planning, and providing ongoing support to children and young people who require long-term intervention. Your work will focus on promoting the well-being, development, and safety of children, with a strong emphasis on partnership working and effective communication.
Key Responsibilities
1. Conducting thorough assessments of children and their families to identify their needs and risks.
2. Developing and implementing child-focused care plans in collaboration with multi-agency partners.
3. Maintaining accurate and up-to-date case records.
4. Providing ongoing support, guidance, and advocacy to children and families.
5. Preparing and presenting reports for court hearings as required.
6. Collaborating with other professionals, including health and education services, to ensure holistic support.
