Job Description
Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.
Key Responsibilities
* Work as a member of the interprofessional team, applying the person-centered approach to care as described by Extendicare's Professional Practice Model;
* Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate and current, communicating changes as needed;
* Implement and evaluate the effectiveness of care/service plans, making adjustments as required;
* Assist with scheduling resident care conferences and participate when applicable;
* Work closely with DOC/DHW to facilitate resident move-in, move-out, and transfers;
* Provide direct resident care, administer medications/treatments, and document care in compliance with regulatory and Extendicare standards and policies;
* Process orders, safely store drugs and narcotics, and administer them according to regulatory requirements and policies;
* Facilitate and provide resident and family education;
* Plan, coordinate, manage, and evaluate resident care plans collaboratively with residents, decision makers, physicians, and the interdisciplinary team;
* Coordinate nursing team assignments to ensure efficient and timely service delivery;
* Respond to significant changes in resident condition or medical emergencies, providing emergency first aid as needed;
* Teach, delegate to, and supervise unregulated care providers;
* Address resident/family concerns, ensuring appropriate action or communication to DOC/DHW/Executive Director;
* Assist in the coordination and monitoring of Infection Prevention and Control, Health and Safety, and Clinical Programs, as well as Quality/Safety Plans;
* Uphold and promote the organization's values and ethics as outlined in the Code of Conduct;
* Perform all other duties as assigned.
Qualifications
* Current registration as a Registered Practical Nurse in the applicable Province/State;
* Experience in a senior's care environment and with unregulated care providers preferred;
* Education and experience in gerontology, rehabilitation, or community/long-term care are assets;
* Strong communication, critical thinking, care coordination, time management, and customer service skills;
* Demonstrated sound clinical knowledge;
* Current Basic Cardiac Life Support and First Aid certification;
* Proficiency with Microsoft Office applications;
* Recent (within 6 months) Vulnerable Position/Sector Screening (VPS).
