Job Overview:
We are currently seeking a highly motivated and compassionate Care Manager to join ateam in Blackpool. The Care Manager will be responsible for overseeing the delivery of high-quality care services to the clients, ensuring their physical, emotional, and social needs are met. Key Responsibilities:
Develop and implement personalized care plans for clients based on their individual needs and preferences
Conduct regular assessments to monitor clients' progress and make necessary adjustments to their care plans
Coordinate and supervise a team of caregivers to ensure the highest level of care is provided to clients
Communicate effectively with clients, their families, and other healthcare professionals to ensure continuity of care
Monitor and maintain accurate records of clients' care and progress
Stay up-to-date with industry trends and best practices in care management
Participate in training and development opportunities to enhance skills and knowledge
Adhere to all company policies and procedures, as well as regulatory requirements
Qualifications:
Experience in Care Management : Previous experience in a managerial or supervisory capacity in the domiciliary care sector is essential.
Qualifications : NVQ/QCF Level 5 in Health and Social Care is desirable or working towards.
Leadership Skills : Proven ability to effectively lead, delegate, and mentor staff.
Communication Skills : Excellent communication skills and a compassionate, empathetic nature.
Organisational Skills : High level of organisational skills with the ability to produce staffing rotas and manage multiple responsibilities.
Flexibility : Ability to work flexibly, including extended hours where necessary, and provide hands-on care during staff shortages.
Positive Attitude: A friendly, supportive demeanour with a genuine passion for assisting those in need.
Collaborator: Willingness to work collaboratively in a fast-paced environment and support your colleagues.
Reliability : Reliable and punctual with a commitment to upholding the highest standards of care.
Driving License : A valid driving license and access to a vehicle are required.
