Deal & Sandwich Primary Care Network (PCN) is seeking a motivated and experienced Paramedic to join our expanding community team. This is a community-based role, working across patients 0homes, care homes, and community settings to deliver high-quality, person-centred care. You will play a vital role in supporting patients closer to home, assessing, treating, and managing health needs while helping to reduce avoidable hospital admissions. The role includes a key focus on delivering the Enhanced Health in Care Homes (EHCH) service across 25 care and nursing homes within the PCN, working collaboratively with residents, families, and care staff to promote wellbeing and independence. Main duties of the job

Undertake pre-arranged and same-day visits to patients in the community and care settings, providing comprehensive clinical assessment, diagnosis, and treatment within professional scope. Conduct regular care home ward rounds under the Enhanced Health in Care Homes (EHCH) framework, completing timely reviews for new residents and hospital discharges, including personalised care planning, treatment escalation, and DNACPR/ReSPECT documentation where appropriate. Collaborate closely with GPs, nurses, pharmacists, social prescribers, care coordinators, and wider PCN and community teams to deliver integrated, patient-centred care. Apply clinical judgment to prioritise caseloads, initiate investigations, and escalate care as required. Support proactive management of long-term conditions, promote self-care, patient education, and preventative strategies, and provide guidance to care home and community staff. Participate in MDT meetings, maintain accurate clinical records, contribute to service development and quality improvement initiatives, uphold professional standards and equality, and engage in continuous professional development and reflective practice.

Deal & Sandwich Primary Care Network (PCN) is a group of five GP practices: Balmoral Surgery, Cedars Surgery, New Golf Road Surgery, Sandwich Medical Practice, and St Richards Road Surgery, collectively serving a population of approximately 48,000 patients. Together these practices collaborate to deliver care in innovative and integrated ways, with the shared aim of helping patients lead healthier, more fulfilling lives. Our team includes a wide range of professionals working together to provide joined-up, proactive care within the community. Job responsibilities

This role offers a unique opportunity for an experienced Paramedic to make a tangible difference to the health and wellbeing of people in our community and care settings. Working at the interface of primary and community care, the post holder will provide high quality holistic care that aligns with the EHCH framework and the broader goals of integrated care.

The Paramedic will play a key role in our Enhanced Health in Care Homes (EHCH) Team, working alongside paramedic colleagues and care coordinators, as well as the wider PCN team, including social prescribers, clinical pharmacists, a pharmacy technician, mental health practitioners and MDT Care Coordinator.

This roles contribution will support continuity, safety, and patient-centred outcomes, strengthening the foundation of proactive, preventative, and compassionate healthcare within our PCN by:

Undertaking both pre-arranged and same-day visits to patients in the community and in care or nursing home settings, providing clinical assessment, diagnosis, and treatment within the scope of professional practice.

Conduct regular care home ward rounds in line with the EHCH framework, delivering proactive and responsive care.

Complete initial reviews for new residents and recent hospital discharges within seven working days, including full clinical assessment, treatment escalation planning, personalised care planning, and DNACPR-ReSPECT documentation where appropriate.

Use sound clinical judgement to assess and prioritise patients, initiate investigations, and escalate or refer to other professionals as required.

Work collaboratively with GPs, nurses, pharmacists, care coordinators, social prescribers, and the wider PCN and community teams to deliver integrated, multi-disciplinary care.

Participate in weekly Multi-Disciplinary Team (MDT) meetings to support care planning for complex or vulnerable patients.

Promote preventative health, self-care, and patient education to improve long-term outcomes and reduce unplanned admissions.

Provide advice and clinical support to care home and community staff to enhance patient safety and quality of care.

Maintain accurate, contemporaneous clinical records on EMIS and other systems to support safe, effective, and auditable care delivery.

Engage in continuing professional development, supervision, and reflective practice to maintain and enhance clinical competence.

Uphold professional standards, equality, and diversity in all interactions, demonstrating compassion, respect, and professionalism.

Contribute to service development, innovation, and quality improvement initiatives across the PCN.

Undertake any other duties aligned with the role, as agreed with the line manager or PCN leadership, to meet service needs.

Person Specification


Qualifications

* BSc in a training programme approved by the College of Paramedics
* Active and relevant professional registration with HCPC
* Completed two-year Consolidation of Learning period as a 'newly qualified paramedic' AND have a further three years' experience as a band 6 (or equivalent) paramedic
* Commit to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of the commencement of the role.
* Evidence of verification against the paramedic primary care roadmap


Knowledge and Skills

* Understands and acts in line with HCPC professional standards for practice
* Compassionate, adaptable, and community-focused approach
* Commitment to providing safe, effective, and person-centred care
* Ability to write comprehensive, accurate clinical notes, implement and evaluate care plans
* Good IT skills including ability to record accurate clinical notes
* Ability to communicate effectively in written, verbal and non-verbal communication with patients, relatives, carers and all members of the multi-disciplinary team
* Confidence working independently in patients homes and community settings
* Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
* Understanding of Confidentiality and the Data protection Act
* Excellent interpersonal, influencing and negotiating skills
* Able to work under pressure and to meet deadlines
* Knowledge of primary care networks and collaborative working


Experience

* Experience in managing the care of patients presenting with undifferentiated undiagnosed conditions and/or a designated caseload, using theoretical and practical experience, completing examinations and assessments, care planning, implementing interventions, referring appropriately, and evaluating outcomes aligned to care plans
* Experience in urgent, emergency, or community-based care
* Strong clinical assessment and decision-making skills
* Experience of working within a multi-disciplinary team
* Experience of working under own direction in a patient focused environment
* Experience of working within a primary care environment or care home environments
* Evidence of working across organisational boundaries within health and social care
* Knowledge of the EHCH framework
* Experience of EMIS
