Carer Support Navigator at Northamptonshire Carers

Location: Hybrid Working, Northamptonshire Carers, Anne Goodman Carers Centre, 123 Midland Road, Wellingbourne, NN8 1LU

Hours: Various hours available

Salary: £12.63 p/h (FTE £24,295.43)

Reporting to: Adult Service Lead – Adult Carer Services


Overview

Our Carer Support Navigators will offer carers assessments and support to carers in Northamptonshire in order to enable the sustainment of the caring role, reduce the impact of caring on carer’s health and wellbeing and to prevent crisis. By ‘carers’ we refer to informal carers who look after a friend or family member with a disability or long-term illness out of a sense of duty and compassion, as opposed to a care worker who is employed in their role. This role includes being part of our Carers Support Line where you will be supporting carers primarily by telephone (both incoming and outgoing calls) and via email alongside working within our community team to provide the best possible support and outcomes for Carers.

Key Responsibilities

* Provide an informed and professional response to carers through a confidential service of carer-centred assessment, support and crisis planning via our Carers Support Line.
* Provision of advice and information alongside emotional support.
* Provide short-term intervention plans for Carers where appropriate, ensuring they are fully supported by effectively managing risk and responsibilities, using a person-centred approach.
* Maintain accurate written and concise computer records in accordance with local policies and data protection and confidentiality legislation.
* Manage own caseload and prioritise tasks and responsibilities whilst ensuring assessments are completed within an agreed timeframe.
* Work in partnership to improve collaboration and coordination to achieve the best possible outcomes for Carers.
* Participate in team meetings, team building, training and activities.
* Participate in regular supervision meetings and seek additional supervision as required.
* Demonstrate awareness/understanding of equal opportunities and other people’s behavioural, physical, social and welfare needs.
* Ensure reasonable care at all times for the health, safety and welfare of yourself and others, and comply with the policies and procedures relating to health and safety and safeguarding within the organisation.
* Undertake developmental tasks and carer awareness-raising as required, including potential fund-raising discussions on a case-by-case basis.
* Carry out any other duties within the broad spirit, scope and purpose of this post, commensurate with the grade of the post.


What We’re Looking For

* Understanding of carers needs and issues
* Previous experience in health care, education, social care or voluntary sector, with partnership and multi-agency working
* Understanding of good practice in information sharing and confidentiality
* Understanding of safeguarding issues in relation to vulnerable people
* Understanding of the Care Act
* Excellent communication skills
* Ability to work under pressure, prioritise tasks and manage time and workload effectively, and work as part of a team
* Positive attitude to carers and service users
* Persistence, assertiveness and flexibility


About You

Ideally you will have a relevant Health or Social Care Qualification, but this is not essential.

You will have the ability to actively listen and empathise with people.

You will have great communication skills both verbally and written.

You are able to organise, plan and prioritise to meet timescales.

You are used to working with IT software, such as MS Word, Powerpoint, Excel and Teams.

You have an awareness of safeguarding for vulnerable adults and Health & Safety at all times.

You will have access to a vehicle.

You may already be working within assessment/support planning, whether this is within a GP surgery/health, social care, support work, or other setting.
