All about the Role
Peterborough City Council is seeking a Post 14 Transitions / Moving On Coordinator to join our Adult Social Care Transitions Service.
This role will coordinate and oversee transition planning at key stages (ages 14, 16 and 17+) to ensure a coordinated plan is developed that identifies how young people can be supported to successfully prepare for and transition into adulthood. The role will focus on those with Special Educational Needs and Disabilities (SEND). The postholder will work closely with children's and adult services, education, health, housing and partner organisations to ensure timely, coordinated and person centred transition planning.
The role focuses on improving outcomes for young people by supporting pathways into post 16 education, employment, training, adult services and, where appropriate, accommodation options beyond the family home.
This is a fixed Term Contract until March 2027

All about You
You will have experience of working with or supporting young people and an understanding of the issues faced by young people with SEND as they prepare for adulthood.
You will be able to:
* Work confidently in a multi agency environment
* Communicate effectively with young people, families and professionals
* Organise and coordinate work across services and partners
* Work with data and outcomes to support service improvement
* Demonstrate a commitment to safeguarding, equality, diversity and inclusion
You will need GCSE Maths and English (or equivalent), good IT skills and the ability to travel across Peterborough.
