An exciting opportunity has become part of the Hospital Discharge Team based at North Devon District Hospital. The team is made up of Social Workers and Social Care Assessors, with the purpose to offer support and assessment for those discharged from hospital on Pathway 2 or 3.
As a Social Worker within the hospital discharge team you will undertake safeguarding enquiry work for individuals in hospital where a section 42(2) enquiry is required. You will also undertake Care Act Assessments for individuals following discharge from hospital. A good working knowledge of the Mental Capacity Act 2005 is required.
We are looking for applicants who are confident in working with people in sometimes difficult situations, their carers and other professionals. You will need to be an effective communicator, both verbally and in writing, be able to prioritise your workload, manage conflict effectively, be flexible and enthusiastic, have effective time management skills and be able to work under pressure. You need to be IT literate and willing to develop your skills. The successful applicant will need to be passionate about working in an inclusive way to achieve positive outcomes for the people they support. In return, you will be part of a friendly, supportive and experienced team with regular supervision, appraisal, regular team meetings and access to learning and development opportunities.
Social Worker Career Pathway
We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for standardising professional capabilities and provides a clear career pathway.
This is an opportunity to provide a clear pathway for your career, enabling you as a practitioner to reflect and identify your own practice and development on the PCF pathway.
If shortlisted for interview, your experience, skills and knowledge will be assessed against the PCF domains to determine appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G). With ongoing learning and support from your line manager, those appointed as a Social Worker can evidence incremental learning and submit an application for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
