Were seeking an experienced Community Case Manager (RMN / RNLD) to join a respected provider of adult and paediatric complex care across Sussex and Hampshire. This is a full-time, MondayFriday (95) post with hybrid working typically two days from home and three days in the community. £50,000£60,000 DOE. As Community Case Manager, youll lead community-based complex care packages for paediatric and adult clients across the counties. Working closely with families, clients and commissioners (Sussex & Hampshire ICBs), youll ensure care is safe, personalised, and consistently high quality. Key Responsibilities Manage and oversee complex care packages across Sussex & Hampshire (home, school, and community settings). Mentor, assess and upskill Nurses and HCAs; complete competencies and support reflective practice. Build strong relationships with commissioners/ICBs, CHC teams and wider MDTs; contribute to reviews and mobilisations. Act as a visible, reliable point of contact for clients, families, and field staff; lead timely escalations and service recovery. Maintain robust governance: safeguarding, risk assessments, care planning, audits, incident review and learning. What Were Looking For RMN or RNLD with active NMC registration. 2 years post-registration experience. Background in community mental health, learning disability, and/or Tier 2 CAMHS (complex needs advantageous). Confident working autonomously across a community patch; strong communication and partnership skills. Full UK driving licence and access to a vehicle.
