This is an exciting opportunity to work within a dynamic Intensive Support Team as part of the wider Learning Disability Community MDT.
We are looking for an experienced nurse who is able to demonstrate evidence of working in a community setting with adults with a learning disability, preferably in crisis situations.
We are a team of clinicians working with service users and their families/carers where there is risk of admission to hospital under the Mental Health Act. We aim to prevent hospital admissions for our service user group, utilising skills in risk assessment & management, positive behaviour assessment & support as well as assessment of mental health that significantly impacts upon day to day life.
The post holder will be required to hold an independent caseload and act as CPA Coordinator, utlising skills in effective communication and in collaboration with the wider MDT.
Main duties of the job
Within a multi professional & multi-agency context apply nursing skills, knowledge and leadership in the provision of a person centred health service that promotes and facilitates the health and well-being of people with learning disabilities who are presenting with risks that puts them at risk of hospital admission under the Mental Health Act.
Provide Health Screening and Health facilitation for those service users with learning disabilities and mental health issues or behaviours which require intensive support and assessment.

Qualifications
Essential criteria
1. Registered learning disability nurse
Desirable criteria
2. First degree level or equivalent competency in a health related subject
3. Leadership qualification
Experience
Essential criteria
4. Experience of working in a multi-professional settings, dealing appropriately with key partner agencies
5. Significant experience post registration nursing.
6. Experience of community working
7. Experience of undertaking assessments of healthcare needs, including mental health or behavioural assessments
8. Experience of working with primary and acute healthcare professionals
Desirable criteria
9. Experience of working in learning disabilities services for people with dual diagnosis or for people with high support needs, including positive behavioural support experience.
10. Experience of delivering training

11. Firm knowledge and understanding of information and communication technology
12. Knowledge of the healthcare complexities associated with learning disabilities
