Community Care Worker Salary: £15.66 per hour (Umbrella) Location: Alfreton (Derbyshire) Client: Derbyshire County council Contract Type: Temporary (6 months) Hours: Full time (37 hours per week) About the Role This role is an exciting opportunity to work as a Community Care Worker (Social Care Practitioner) within Derbyshire County Council’s South OT Team, based in Swadlincote. You will support adults in the community to maximise their independence through holistic assessments, equipment provision, and housing adaptations, while working closely with Occupational Therapists, health partners, and housing providers. The role requires a person-centred, strength-based approach, ensuring safe and sustainable outcomes for individuals, carers, and families. You will be part of a supportive team with regular supervision, peer support, and opportunities for development. Key Responsibilities Work with adults, families, and carers to maximise independence and support daily living. Undertake holistic assessments of needs, including Occupational Therapy interventions. Prescribe and arrange equipment solutions, minor and major adaptations. Liaise with housing providers, health professionals, and community partners. Contribute to safeguarding vulnerable adults. Maintain accurate and timely records using council systems (Mosaic desirable). Commission services in response to assessed need. Take a strength-based, person-centred approach in all work. Experience & Knowledge Proven experience of working with adults in the community in a Local Authority setting. Knowledge/experience of equipment and adaptation solutions. Experience of assessing activities of daily living. Understanding of physical, learning disabilities and/or mental health conditions. Knowledge of safeguarding principles and practice. Awareness of statutory legislation underpinning Occupational Therapy assessments. Experience of partnership working across agencies (Health, Housing, Voluntary sector). Understanding of positive risk-taking and its role in adult social care. Skills & Competencies Strong communication and interpersonal skills. Ability to build positive relationships with service users, carers, and professionals. Good IT and record-keeping skills (Mosaic desirable). Organisational skills with the ability to prioritise caseloads. Reflective practice and commitment to supervision and development. Ability to work independently and as part of a team. Qualifications NVQ Level 3 in Care (or equivalent) – essential* NVQ Level 4 in Care (or equivalent) – desirable* General academic qualifications considered desirable. (*Candidates without these qualifications must be willing to work towards them once in post.) Other Requirements Ability to travel for home visits across South Derbyshire.
