Overview
Treatment Room Nurse – Burnham, Berrow & Highbridge (Somerset). 12-month fixed term contract, 34 hours per week, across Burnham Medical Centre, Love Lane, Burnham-on-Sea and Highbridge Medical Centre, Pepperall Road, Highbridge. Salary £36,784 per annum (pro rata for part time hours).
Join a dedicated Nursing Team at Symphony North, providing optimum care relevant to the practice population in practice or the home environment. You will support patients to be healthy, monitor long-term conditions, and contribute to health prevention and screening activities in line with the Symphony Healthcare Services objectives. You will demonstrate safe, clinical decision-making, assessment and expert care.
Responsibilities
* Assist the whole practice team to deliver effective management of patients and their needs.
* Manage nursing care of patients with chronic disease, using recall systems suitable to practice needs; input data accurately for recalls and screening, including contract data input.
* Contribute to the development of policies, protocols and guidelines within the practice.
* Support the practice team with implementation of new initiatives in chronic disease management services.
* Encourage development of integrated services within Primary Care and multidisciplinary working.
* Recognise opportunities for partnership with the Primary Health care team.
* Implement and evaluate individual treatment plans for patients with known long-term conditions.
* Prioritise health problems and intervene in complex, urgent or emergency situations, including initiation of effective emergency care.
Clinical Skills & Chronic Disease Management
* Wound care, dressings and general treatment room skills
* Urinalysis, spirometry, contraception, family planning and sexual health
* Cytology, Warfarin (INR) monitoring, coil removal, Thyroxine register
* Ear syringing, ECG, blood pressure and weight measurement
* Diet and lifestyle advice; immunisations (childhood, flu, pneumonia, pertussis, shingles, etc.)
* Injections (e.g., B12, Triptorelin, Zoladex, Depo), travel vaccinations
* Stock checking and ordering of vaccines
* Chronic disease skills include hypertension, diabetes, asthma/COPD, mental health, CHD
Qualifications
Essential
* Registered nurse
* Willingness to learn and acquire new skills
* Insight into current issues in nursing and the NHS
Desirable
* Post-registration study in topics allied to general practice
Experience
Essential
* Evidence of appropriate knowledge base; interest and basic knowledge in long-term conditions
* Understanding of audit; Primary Care experience
Desirable
* Post-registration GP practice experience; experience using GP clinical systems (EMIS preferred)
* Life support certifications; awareness of health and safety issues
* Mentorship training

* Good interpersonal, verbal and written communication
* Motivation, reliability and teamwork
* Knowledge of long-term care needs and health promotion strategies
* Clinical governance awareness in primary care and accountability of own role
