Who You Are You are an experienced and dedicated social worker with a professional qualification in social work and current Social Work England (SWE) registration. You possess a genuine commitment to working with children, young people, and adults with disabilities. You understand the nuances of transitions from childhood to adulthood and have expertise in both Children and Adult legislation. You are prepared to travel throughout Worcestershire, working across multiple agencies to support young people in their journey to adulthood. What the Job Involves You will work within the All Age Disability (AAD) Service as part of the Young Adults Team, supporting young people with disabilities aged 16-25. This involves preparing them for adulthood in areas such as education, employment, independent living, community inclusion, and health. You will collaborate with Worcestershire Children Services, Adult Services, SEND, health colleagues, schools, and colleges. Your responsibilities include safeguarding and promoting welfare, assessing risks and needs, coordinating services, and ensuring accurate record-keeping and reporting. Flexibility in working hours will be required to provide high-quality services, often outside of core office hours.
