We have an exciting opportunity for a qualified Social Worker based at Trinity Hospice for 18.5 hours per week.
The successful candidate will work alongside Trinity Hospice inpatient and community Multi-Disciplinary teams to provide timely and robust care and support to individuals and families in need. You will need to be empathetic and compassionate; and have a passion for supporting individuals and families in end of life care planning. You will be part of a wider health and community social care team and will have access to a supportive and enthusiastic team members and managers; and will have access to regular supervision and team development opportunities.
You will be supporting Blackpool, Fylde and Wyre residents and will be required to work with Lancashire County Council colleagues, managers and use LAS recording systems.

Responsibilities
* Work with Trinity Hospice inpatient and community Multi-Disciplinary Teams to provide timely and robust care and support to individuals and families in need.
* Collaborate with Lancashire County Council colleagues and managers; use LAS recording systems.
* Contribute to end of life care planning and holistic support within a health and community social care context.
Requirements / Qualifications
* Qualified Social Worker (Social Work degree and applicable registration).
* Empathetic, compassionate approach with a passion for end of life care planning.
* Ability to work with Blackpool, Fylde and Wyre residents and coordinate with multidisciplinary teams.
* Willingness to engage in regular supervision and participate in team development opportunities.
