Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe, well and out of hospital as well as assisting to reduce the demands on general practice.

Responsibilities
Main Duties
* Clinical: Support the delivery of clinical pharmacy services across the PCN, to include:
* liaising with clinicians and administrative staff to answer medicines-related queries within the Technician’s scope of competence or refer to the most suitable healthcare professional where appropriate;
* assist the Clinical Pharmacists with Structured Medication Reviews (SMR) following agreed protocols, providing background information, arranging monitoring tests, follow-up after review, and conducting clinics (telephone and face-to-face), including domiciliary and Care Home visits;
* provide medicines reconciliation for patients when registering or transferring into primary care, liaising with patients and other providers to ensure appropriate medication;
* assist with medication safety monitoring systems including drug monitoring of medicines prescribed under shared care;
* participate in quality improvement and clinical audits, including data collection and safety tasks;
* provide lifestyle advice and signposting to address public health and social care needs; support antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.
* Technical and administration: Improve and implement efficient ordering and repeat prescribing processes, including electronic prescribing and eRD; support PCN with prescription processing and liaison with community pharmacy regarding changes and out-of-stock medications; provide advice on safe handling of medicines; maintain the practice formulary; monitor prescribing against PCN, local and national policies; assist with CAS/MHRA alerts and guidelines; support QIPP, QOF and locally commissioned enhanced services; help develop practice policies for CQC requirements.

Qualifications and Experience
Essential
* Professional registration with the General Pharmaceutical Council (GPhC).
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
* Experience as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy.
* Evidence of continued professional development.
Desirable
* Experience working with GP surgeries in this role.
* ACT qualification; experience influencing and persuading partners and stakeholders.
* Knowledge of primary care networks, NHS priorities and evidence-based medicine.
* Experience using GP clinical systems (e.g., SystmOne) and Ardens; patient counselling; supervisory experience.

* Strong computer literacy and attention to detail; able to manage resources and meet deadlines.
* Excellent communication with patients and healthcare professionals; adaptable to different audiences.
* Knowledge of data protection and confidentiality; analytical thinking to solve problems; negotiation and influencing skills; ability to work independently and in a team.

* Experience with GP Clinical systems (SystmOne, Ardens); experience in patient counselling; supervisory experience.
