Overview
We are looking for enthusiastic and motivated Community Staff Nurse to join the community IV Team. The post-holder is responsible for providing the ongoing nursing care for patients requiring Intravenous (IV) therapy and supporting the virtual ward in the community. The post-holder will carry out all forms of clinical care associated with IV therapy and assessment of care needs including the individual development, implementation and evaluation of programmes of care for patients. The successful candidate will provide nursing care to patients within their own homes and in clinic settings.
Responsibilities
* Provide nursing care to patients within their own homes and in clinic settings, including IV therapy and assessment of care needs.
* Develop, implement and evaluate programmes of care for individual patients.
* Maintain effective communication and interpersonal skills; work flexibly and collaboratively to meet the needs of the service.
* Support the virtual ward in the community and participate in development and mentorship within the team.

* Demonstrated commitment to safeguarding and patient-centred care.
* Vaccination requirements: role falls under government mandate; evidence of receiving both doses or medically exempt, where applicable.

The role may involve cross-site working across the Black Country Provider Collaborative Trusts.
