The successful applicant will be an NHS employee and may therefore have contact with vulnerable service users. We provide clinical pharmacy services to the Initial Response Service across Cornwall. Your base location is negotiable, though travel to other sites will be required. Working patterns, including a rolling weekend rota, are flexible and open to discussion for the right candidate. This role will require working independently and collaboratively within the wider pharmacy team.

This post will support hospitals within Cornwall. Base location can be negotiated but there will be a requirement to travel to other peripheral hospitals.

Responsibilities

* As a Medicines Optimisation Pharmacy Technician within the crisis service, support medicines reconciliation, prescription reviews, discharge facilitation, ward-based dispensing, medication recycling, patient counselling, and audits.
* Provide a comprehensive medicines management service to patients with the Initial Response Service and support the Mental Health hospital inpatients, including medicines reconciliation, prescription review and endorsement, recycling of medication, supporting self-medication and, where necessary, patient and carer counselling.
* Participate in multi-disciplinary activities such as ward handover meetings and facilitation and preparation of discharge medication with handover information to other healthcare professionals if required.
* Work with the pharmacy team to deliver clinical pharmacy and safe systems of working with medicines across mental health sites, with scope to work with patients directly in the community.
* Supervise and support education and training activities for student technicians, student pharmacists and pre-registration pharmacists.
* Support hospitals within Cornwall; base location negotiable with travel requirements to peripheral hospitals.


Requirements and Qualifications

* Base location negotiable; willingness to travel to other sites as required.
* Experience as a Medicines Optimisation Pharmacy Technician within NHS settings preferred.
* Ability to work independently and within a wider pharmacy team.
* Commitment to delivering safe medicines management and patient-centred care.
