Workingcollaboratively across primary care and our Neighbourhood Health Partnership,you will help reduce health inequalities, support people to live independently,and improve overall health outcomes through personalised care and socialprescribing.

Job responsibilities
SpecificResponsibilities
Establish strong links and excellent workingrelationships with primary and secondary care colleagues, VCSE partners andother organisations.
Accept referrals and access how far a patientshealth and wellbeing needs can be met by services and other opportunities inthe community.
Manage and prioritise your caseload reflecting yourpatients needs.
Together produce a simple personalised care andsupport plan to address their health and wellbeing needs by introducing orreconnecting people to community groups and statutory services, includingweight management support, smoking cessation, taking up employment, training,welfare support (personal health budget) and sign posting where it matters tothe person.
Sharing the knowledge and promoting the benefitswith colleagues in the PCN on other services available in the local communityand how this can be accessed
Support the VCSE organisation and community groupsto become more sustainable, sharing intelligence regarding gaps or issuesidentified in local provision.
Gather data to build on evidence base about thebenefits of social prescribing.
Ensure data is captured in the clinical systems.
Attend the peer support networks run by NHS Englandand NHS improvement at ICS level.
Be part of the Integrated Neighbourhood Teams tosupport patients and to link in with other organisations.
Have a proactive approach, the ability to work onyour own initiative and within a team environment.
Person Specification
* Completed the eLearning for Health online programme Social Prescribing leaning for link workers
* Enrolled, in progress or qualified from appropriate training as set out by the Personalised Care Institute.
* Experience of working within a community, social care, or voluntary sector
