We are looking for Community Staff Nurses for our School Nursing teams in the following areas:
Lowestoft and Waveney - 30 hours (TTO)
North & East Ipswich and Coastal - 37 hours (TTO)
South & West Ipswich - 37 hours (TTO)
Central and South Suffolk - 37 hours (TTO)
West Suffolk - 37 hours (TTO)

You will:
* support the delivery of the Healthy Child Programme 5-19 yrs (HCP) by directly supporting the needs of children, young people, and their families
* through multiagency working, effectively safeguard children and young people, which will include participation in safeguarding meetings and contributing to assessment processes
* where required, be the key contact with the family and liaise with other professionals to provide a joined-up response
* have a key role in delivering the Healthy Child Programme
* support health needs of school age children and their families through a Public Health approach
* promote health and wellbeing to ensure each child and young person is supported to reach their potential
* delivering meaningful support to children and young people to achieve positive outcomes.

* current Nursing and Midwifery Council (NMC) Registered Nurse.

Work with children, young people and families using Suffolk Signs ofSafety (SOS) and Wellbeing framework.
Apply an integrated, multi-agency approach to supporting and empoweringchildren, young people and their families.
To ensure a strong focus on prevention, early intervention andidentification of need, using your experience, knowledge and skill to determinethe level of intervention required.
Work in accordance with regulatory and statuary documents, legalframeworks, NICE guidance and Suffolk County Council policy and procedures.
Actively build relationships and networks with other professionalgroups and services in the locality to deliver a holistic service to children,young people and their families.
To work with the community and ensure the active participating ofchildren, young people and families in service planning and delivery thatrespond to local needs and reflects the diversity of the community and theinclusion of vulnerable people and those with additional needs.
To identify risk and safeguarding concerns and escalate where required,undertaking appropriate referrals, including CAF.

Plan and deliver universal and targeted provisionwhich focuses on improving outcomes for children, young people and theirfamilies.
To work alongside children, young people andfamilies where there are early signs of social, emotional, health orbehavioural issues to bring about sustainable improvement.
Identify early children and families whereadditional preventative interventions could reduce their risks and improvefuture health and wellbeing.
To respond to needs identified within the family byidentifying and delivering evidence -based interventions and escalating asrequired.
A thorough understanding information and advice tosignpost for children, young people and families to relevant universal andspecialist services in the local area and beyond, empowering families to helpthemselves.
Support children, young people and their familiesto maintain their own good emotional health and wellbeing, finding their ownsupport network from friends, family and the community.
Engage in individual and group supervision.
To mentor and/ or supervise junior colleagues orstudents.
To contribute to ensure successful completion ofquality assurance processes within the team.
To be responsible for individual performance andthe quality of data produced.
Use feedback to inform service delivery and staffdevelopment.
