Job Overview: We are seeking an experienced and proactive Band 6 Occupational Therapist to join the community rehabilitation team. This role involves home visits within the local area, so access to your own vehicle is essential. The successful candidate will carry out holistic assessments and provide interventions to support patients in regaining independence and reducing reliance on long-term domiciliary care. Key Responsibilities: Conduct comprehensive functional assessments in patients' homes Deliver goal-focused interventions aimed at increasing independence Identify risks and recommend appropriate adaptations or equipment Support discharge planning and help prevent hospital readmissions Maintain accurate and timely clinical documentation Liaise effectively with families, carers, and multidisciplinary teams Key Requirements: HCPC registered Occupational Therapist Minimum 6 months of NHS experience at Band 6 level or higher Previous community rehab experience preferred Confident working independently in patients' homes Access to own vehicle and full driving licence Additional Information: Community-based role - local travel required Full-time preferred (Monday-Friday, 08:30-16:30); part-time may be considered Immediate start available for the right candidate How to Apply: Please send your updated CV to or contact us on 0333 121 20 10, quoting reference AA/10479/OTB6 .
