Band 6 Occupational Therapist - Community Rehabilitation
(Minimum 6 months NHS relevant experience is mandatory)

Pay Rate: £21 - £25 per hour PAYE (DE)

Job Overview:
We are seeking an experienced and proactive Band 6 Occupational Therapist to join the community rehabilitation team. This role involves home visits within the local area, so access to your own vehicle is essential. The successful candidate will carry out holistic assessments and provide interventions to support patients in regaining independence and reducing reliance on long-term domiciliary care.
Key Responsibilities:
* Conduct comprehensive functional assessments in patients' homes
* Deliver goal-focused interventions aimed at increasing independence
* Identify risks and recommend appropriate adaptations or equipment
* Support discharge planning and help prevent hospital readmissions
* Maintain accurate and timely clinical documentation
* Liaise effectively with families, carers, and multidisciplinary teams
Key Requirements:
* HCPC registered Occupational Therapist
* Minimum 6 months of NHS experience at Band 6 level or higher
* Previous community rehab experience preferred
* Confident working independently in patients' homes
* Access to own vehicle and full driving licence
Additional Information:
* Community-based role - local travel required
* Full-time preferred (Monday-Friday, 08:30-16:30); part-time may be considered
