Responsibilities Conduct comprehensive assessments to determine the needs of clients and their families. Develop and implement individualized intervention plans in collaboration with clients. Provide counseling and support to clients in coping with various life challenges. Advocate for clients' access to community resources and services. Facilitate support groups and educational workshops for clients and their families. Maintain accurate and confidential client records and documentation as required by law and organizational policies. Collaborate with other professionals, including healthcare providers, to ensure integrated care for clients. Requirements Master's degree in Social Work from an accredited institution. Current state licensure as a Social Worker. A minimum of two years of experience in a social work setting. Strong knowledge of community resources and support services available to clients. Excellent communication and interpersonal skills, enabling effective interaction with clients and colleagues. Demonstrated ability to work independently and manage a caseload effectively.
